Registered Nurse 

The Float Nurse will ensure the health and safety of the elders is being met through assessments, provision of services, care planning, and supervision of other caregivers in the home. The Float Nurse will communicate with healthcare providers, families, elders, and staff to ensure the highest practicable social, physical, emotional, and psychosocial needs are being provided. The Float Nurse must be flexible and willing to work in any location as directed by their supervisor. The Float Nurse will support the nursing needs of the facility by filling in and supporting as needed and as directed daily.

Essential Functions:

​

Administration of medications and treatments
Completion of assessments as needed
Always oversee direct care staff, ensuring the plan of care is being followed and recommending changes to the care plan as necessary
Provide support to other RN, LPN & and NARs to ensure high quality care and adherence to facility policy and procedure
Enforce program, operational, and personnel policies and procedures
Protects elders and staff by auditing and providing education on infection-control policies and protocols, enforcing medication administration, storage procedures, and controlled substance regulations and others as directed by the Nurse Manager or DON
Maintains a cooperative relationship among health care teams by communicating information, responding to requests; building rapport; participating in team problem-solving methods
Promotes elders; independence by establishing patient care goals; teaching and counseling elder, friends, and family and reinforcing their understanding of disease, medications, self-care skills.
Work with other healthcare providers such as physicians and therapists to ensure high levels of quality care and services are being always provided.
Assist with RN or LPN duties as requested by Nurse Manger or DON
Must be agreeable to sudden changes and emergent needs based on daily needs of facility
Other duties as assigned


Physical Demands:

 

Must be able to move throughout the workday and throughout the building.
Must have adequate vision and hearing to meet the requirements of this position.
Must be able to cope with the mental and emotional stress of the position.
While performing duties of this job, employee is regularly required to use hands and fingers to examine clients, handle or feel objects, tools, or controls, and reach with hands and arms.

Minimum Qualifications:

​

Must be able to work in federal health program
Must be Registered Nurse on the MN Board of Nursing Registry
CPR/First Aid
Preferred Qualifications:

Long Term Care Experience
Experience with electronic medical records
Transitional Care Unit experience

Nurse and Patient on Wheelchair

Licensed Practical Nurse

The Licensed Practical Nurse, LPN will primarily provide routine care, observe patients’ health, assist
physicians and other healthcare providers, and communicate instructions to patients regarding
medications and other treatments and functions of care.
Assist in development of treatment plan for each elder and ensure the care strategy addresses the long-term needs of the aging process
Administration of basic nursing care
Administer medications
Operate and monitor medical equipment
Observe the elder and record observations
Always oversee other care staff, ensuring the plan of care is being carried through
Advocate for clients’ best interests and serve as an educator to caregivers, elders, and families
Work with other healthcare providers such as physicians and therapists to ensure high levels of quality care and services are being always provided.
Communicate any change in elder & condition to appropriate staff, health care providers, and family.

Minimum Qualifications:

Certified Nursing Assistant

To provide basic elder care under the supervision of a licensed nurse and assists in the maintenance of the unit

.
Essential Functions:


Assists with personal care under the supervision of the household nurse
Perform basic household functions including spot cleaning of closets, high stand, beds, mattresses, side rails, drawers and over bed tables
Assist co-workers in tidying dayrooms, utility rooms, spa, and any other areas around the household
Provides PROM to elders
Serves, assists and feeds residents at mealtimes
Charts all pertinent information in appropriate place or flow sheet
Understands and implements infection control standards especially hand washing and linen handling
Adheres to Resident Bill of Rights
Observes confidentiality of residents, employees and facility information
Other duties as assigned


Physical Demands:

 

Be able to deal effectively with residents who may be physically and mentally abusive, have a good understanding of basic body mechanics, able to lift/carry up to 50 pounds, and be able to push/pull up to 75 pounds. While performing the duties of this job, the employee is frequently
required to stand, bend, and walk. The employee is occasionally required to lift up to 50 pounds, sit,
climb, stoop, kneel, crouch, and crawl.

Minimum Qualifications:

​

Registered nursing assistant on Minnesota registry
Able to speak, read, and write the English language
Proficient technical and communication skills
Preferred Qualifications:
Experience in long term care and transitional care unit .
